Leading Into New Communities (LINC)

• Mission: Empowering individuals with criminal histories or youth that are at-risk to become serious offenders of the justice system to make positive life choices, enabling them to become productive members of the our community.
• LINC provides services that meet the immediate needs of ex-offenders, such as shelter, food, clothing and employment.
• LINC also refers clients to other social service agencies that provide services to further facilitate their reintegration into the community thus Leading Into New Communities.



Coastal Horizons Center

 

• To promote choices for healthier lives and safer communities by providing professional assistance to those in need of prevention, crisis intervention, criminal justice alternatives, community outreach, substance abuse and mental health treatment services.



Southeastern Center for Mental Health

• Southeastern Center shall ensure an accessible, flexible, and responsive system of care resulting in an improved quality of life for consumers of mental health, developmental disability, and/or substance abuse services.



New Hanover Division of Social Services

• The North Carolina Department of Health and Human Services, in collaboration with its partners, protects the health and safety of all North Carolinians and provides essential human services.



New Hanover County Health Department

• The mission of the New Hanover County Health Department is to assure a safe and healthy community.
• The vision of the New Hanover County Health Department is Healthy People, Healthy Environment, Healthy Community.




• Mission: to provide a full spectrum of primary and preventative health care services (including essential ancillary and enabling services) to medically underserved populations, in New Hanover and surrounding counties in Southeastern North Carolina. Patients receive services regardless of the individuals or families ability to pay. Our service is designed to cover prenatal, pediatric, adolescent, adult and geriatric life cycles.



Wilmington Health Access for Teens (WHAT)

• Mission: To improve the health of the youth of our community by ensuring access for all to quality physical and mental health services and by providing prevention, education and outreach services to promote optimal health.



Cape Fear HealthNet

• Goals:

1. To expand the safety net to provide medical homes for primary care
2. To create a centralized system that standardizes eligibility, redistributes patients throughout the safety net based on acuity and capacity, and refers them for specialty care
3. To provide case management of patients with chronic disease, teaching them to manage their symptoms and avoid unnecessary medical care
4. To create a network of volunteer physicians for referrals from the safety net
5. To create a network of pharmacy solutions to ensure patients can afford medications



Feast Down East-UNCW

• FEASTDOWNEAST is an initiative established to create a fully integrated local food system that helps small and limited resource farmers gain access to markets such as restaurants, grocers, schools and hospitals.



Food Bank of Central & Eastern NC

• Mission: To harness and supply resources so that no one goes hungry in central and eastern North Carolina.

• The disAbility Resource Center provides services to people with disabilities of all ages and all types of disabilities and those who support them
